CREMATION SERVICES APPLICATION GUIDELINES

[For Hindus]

  1. Any person can apply for cremation services on behalf of the family and the next of kin of the deceased. Next of kin are spouse, children, parents and siblings. A neighbour, a relative or a friend can also apply.

  2. Applicant to fill the form [Request For Cremation Services for Hindus] available from http://www.shm.or.tz/crematorium upload the requested documents and submit online.

  3. Alternatively, the form can be downloaded from the website, filled and taken to the office of the Secretariat of Shree Hindu Mandal, Chusi Street. 

  4. Note that the Applicant will have to follow all the legal procedures before applying for cremation services. The legal process [obtaining KIBALI CHA MAZISHI] is not the responsibility of Shree Hindu Mandal - Welfare.

  5. KIBALI CHA MAZISHI is issued by a Registered Medical Officer after certifying death. If the deceased was a non-Tanzanian, a Consent Letter from the respective High Commission/Embassy maybe required at the time of applying for cremation services. This is depended on the requirements of the respective High Commission/Embassy.

  6. The date and time of cremation will depend on the availability of slots on that day and will be decided by the General Manager. The Applicant will get a confirmation call.

  7. Preferred funeral cortege departure timings are 10am in the morning and 2pm in the afternoon. 

  8. For preparation of the deceased body on the day of cremation, the Applicant must report with minimum 3 persons to the mortuary for bathing ritual of the deceased along with the items required for the bathing ritual. The bathing time is 2 hours before the funeral cortege time.

  9. Applicant shall take note of the items required for cremation services as below; 

    1. Bathing ritual at mortuary

    2. Bathing soap 

    3. Small napkin

    4. Bath towel 

    5. Perfume

  10. For males: Trouser, shirt and undergarments, shaving items [if desired] 

  11. For females: Sari, Blouse and undergarments, makeup kit and ornaments. If the deceased was suhagan then 2 red pieces of cloth, each of 2.5 meters length and Chandlo 

    1. Tilak, Chandlo Kit and Kanthi

  12. The SHM Welfare shall provide free 2 white pieces of cloth, each of 2.5 meters length

INFORMATION OF PROCESSES FROM PRESUMED DEATH TO CREMATION [For Hindus only]

  1. Any person can apply for cremation services on behalf of the family and the next of kin of the deceased. Next of kin are spouse, children, parents and siblings. Applicant can also be a next of kin, a neighbour, a relative or a friend.

  2. The below processes serve as a guideline only for the Applicant. Shree Hindu Mandal Crematorium Department is not responsible nor does it facilitate these services:

  3. If the person is presumed dead at home – either the certification needs to be done at Home [any doctor from any hospital or health centre could be called to go and certify at home and issue a KIBALI CHA MAZISHI] After acquiring KIBALI CHA MAZISHI from the doctor, a call has to be made to the Crematorium Department of Shree Hindu Mandal or any other mortuary facility to safely transport the body.

  4. Shree Hindu Mandal Hospital provides ambulance service at a cost. Please contact 0222114991-4. NOTE that the ambulance is not used for carrying dead bodies.

  5. If the person is presumed dead at home – but no doctor is available to certify at home, then an ambulance from any hospital or health centre can be called and once the person is brought to the Hospital, the doctor on duty will have to certify and issue KIBALI CHA MAZISHI.

  6. The KIBALI CHA MAZISHI is issued by any doctor after determining the CAUSE OF DEATH. 

  7. If the body is brought to Shree Hindu Mandal Hospital where the dead person has never been treated, then the doctor on duty will not be able to issue KIBALI CHA MAZISHI without knowing the CAUSE OF DEATH. 

  8. If the doctor at Shree Hindu Mandal Hospital is convinced by the medical records submitted of the dead person then KIBALI CHA MAZISHI can be issued or else the body has to be sent to The Muhimbili National Hospital for the post mortem to identify the CAUSE OF DEATH and subsequent issuance of KIBALI CHA MAZISHI.

  9. Alternatively, the Applicant may approach the Ward Executive Office [Ofisi ya Mtendaji wa Kata] whereby they issue a letter to RITA who will then subsequently issue the KIBALI CHA MAZISHI.   10. Cremation Permit will be issued after acquiring KIBALI CHA MAZISHI.

  11. For Non Tanzanians, subject to the Nationality of the deceased, consent of the respective Embassy or High Commission may be required before cremation.

  12. The deceased body may be stored at a mortuary at any hospital or may be transported to SHM Mortuary with appropriate documents including KIBALI CHA MAZISHI. Our Crematorium Department volunteers are available 24/7 for this service. Their contacts are available on the website. 

  13. Once the above processes are cleared, the Applicant shall visit www.shm.or.tz and fill the online form, upload the documents and wait for the confirmation call on the date of cremation (Allow 12 hours response time)

  14. After cremation, the Applicant or the family of the deceased may go along the shores of Indian Ocean to scatter the deceased ashes using the permit bearing reference DSM/ILALA/A.16/15/VOL.III/97 of 18 Jan 2013. Be mindful of private beaches and government sensitive areas. Our recommendation is to use The Slipway or the Tanganyika Swimming Club in Dar Es Salaam. 

  15. Shanti Path and other rituals may follow depending on the arrangements made by the deceased family.

  16. Cremation services are availed free of cost to the Hindu Community by the Welfare Board of Shree Hindu Mandal. However, this service provision has high costs. The community acknowledges and appreciates the free services provided by few volunteers, some donors who fund the day-to-day maintenance & operations. However, that is never enough and the Welfare Board is humbly appealing for donations or contributions from those who use the services as well as other well-wishers of their own free will. The donations/contributions can be deposited or transferred directly into our bank [account number 24110004625 held at the NMB Bank PLC, Ohio Street/Ali Hassan Mwinyi Road, Dar es salaam [SWIFT NMIBTZTZ] in the name of SHREE HINDU MANDAL- WELFARE. All donations and contributions are acknowledged by official receipts and the funds are used towards running of the Crematorium.

  2. Applicant to fill the form [Request For Cremation Services for Non Hindu] available from http://www.shm.or.tz/crematorium upload the requested documents and submit online.

  3. Alternatively, the form can be downloaded from the website, filled and taken to the office of the Administrative Secretariat of Shree Hindu Mandal, Chusi Street.  

  4. Note that the Applicant will have to follow all the legal procedures before applying for cremation services. The legal process [obtaining KIBALI CHA MAZISHI] is not the responsibility of Shree Hindu Mandal Crematorium 

  5. KIBALI CHA MAZISHI is issued by a Registered Medical Officer after certifying death. A Consent letter from respective High Commission/Embassy is required at the time of applying for cremation services if the deceased was a non-Tanzanian.

  6. The date and time of cremation will depend on the availability of slots on that day and will be decided by the General Manager. The Applicant will get a confirmation call.

  7. If funeral cortege service is required, departure timings are 10am in the morning and 2pm in the afternoon. 

  8. The fee for using the Shree Hindu Mandal cremation service is maximum TZS 2,500,000/- [Two Million Five Hundred Thousand only] payable in advance to our bank account no. 24110004625 held at the NMB Bank PLC, Ohio Street/Ali Hassan Mwinyi Road, Dar es salaam [SWIFT NMIBTZTZ] in the name of SHREE HINDU MANDAL- WELFARE.
